Abstract

Purpose

This paper's aim is to evaluate understanding and knowledge of the Adult Support and Protection (Scotland) Act 2007 in a sample of community nurses working in learning disability services in Scotland.

Design/methodology/approach

Ten community nurses who worked in learning disability services in one NHS area were tested at two time points, four months apart using a questionnaire designed for this study by researchers and practitioners. Level of previous national training in the Adult Support and Protection Act and length of time working with people with learning disabilities were recorded. Three domains of adult protection were included in the questionnaire: Principles of the Act and definitions; Adults at risk of harm; Protection, assessment, removal and banning orders.

Findings

Questionnaire scores varied widely overall and across the three domains. There was no correlation between individual scores and training or length of work experience. The level of knowledge was below what might have been expected for this group, given the level of training and experience. Carefully designed verification of the impact of nationally approved adult support and protection training is needed.

Originality/value

There is an absence of research in evaluating the impact of the approved Scottish Government training materials on staff knowledge and understanding of the 2007 Act, with staff attendance being taken as the main measure of training compliance. This was a small scale pilot study and recommendations are made for the scope and methods of evaluation.

Abstract

The literature examining the behaviour of environmentally conscious consumers has focused mainly on the examination of non‐product specific environmental knowledge and attitudes or environmental knowledge and attitudes in relation to single product lines. Employs the constructs of product‐line‐specific environmental knowledge and attitudes, that is knowledge of and attitudes towards the green products and their impact on the environment. Presents the results of an exploratory study examining the relationship between product‐line‐specific environmental knowledge and attitudes for multiple green product lines, testing hypotheses generated from the literature, utilizing a questionnaire measuring self‐reports of environmental knowledge and attitudes. The results show no direct relationship exists between product‐line‐specific environmental knowledge and attitudes, and that consumers do not simply believe that a green product is good for the environment without also knowing how the product impacts on the environment.

Abstract

Purpose

Historically, children with autism spectrum disorder (ASD) were sometimes diagnosed with schizophrenia or major psychosis. Although significant advancements in the process of differential diagnosis have been made since 1950s, there still exists a problematic delay in diagnosis due to overlap of symptoms. Negative symptoms of schizophrenia can mimic the social difficulties and stereotyped behaviors characteristic of ASD, whereas positive symptoms of schizophrenia can be perceived as restricted and repetitive behaviors, complicating the diagnostic process. The purpose of this paper is to present two clinical cases that highlight the complexities in differential diagnosis of early psychosis, schizophrenia and ASD.

Design/methodology/approach

Two females, 14 and 16 years of age, were referred to a free screening clinic in Southern California to be assessed for possible ASD. Both females were referred because of the presentation of restricted and repetitive behaviors and social communication difficulties. Both females and their families were administered a battery of measures to ascertain the youths’ cognitive functioning, adaptive living skills and severity of autism-related behaviors.

Findings

The 14-year-old presented with early-stage (prodromal or at-risk mental state) psychosis; 16-year-old met criteria for schizophrenia. Both were referred to clinics specializing in treatment for psychosis and/or schizophrenia. Neither met criteria for ASD.

Originality/value

More published studies are needed on the overlap of symptoms between ASD and schizophrenia to help prevent diagnostic overshadowing of autistic symptoms and promote treatment during the early stages of psychosis. This is particularly important given the strong evidence that early treatment for psychosis improves social, cognitive and functional outcomes.

Abstract

Purpose

Demographic changes involving a worldwide ageing population and later retirements produce a gradual ageing of the workforce and major concerns about how ageing may influence the workplace. This paper aims to provide evidence relating to older workers in healthcare settings in Australia.

Design/methodology/approach

Using a secondary quantitative dataset, the authors sub-sampled a group of workers in the healthcare sector. We used linear regression arrangement with hypotheses focused on the assessment of the significance of interaction or moderation effects relating to job characteristics and age on employee satisfaction.

Findings

The authors note that older workers' job satisfaction is negatively influenced by poor perceptions of job security and autonomy in how their work is carried out. Ensuring that older workers stay in the healthcare workforce is imperative as the work force ages. This paper shows that managing their job security and offering them work autonomy enhance their job satisfaction.

Research limitations/implications

The use of a secondary and cross-sectional dataset has some limitations relating to endogeneity, although these have been managed and assessed. The paper is based on a representative sample of Australian workers, and is thus generalisable within the Australian context, and will be informative elsewhere.

Practical implications

The focus on elements of flexibility for older workers (enhanced autonomy) and clearer job security elements is of practical relevance in the management of older workers.

Social implications

As the overall population ages, supporting older workers in their careers will be of increasing importance. In sectors with a disproportionate share of older workers, like health care, this imperative will come sooner, and the benefits of getting arrangements right be will higher.

Originality/value

No other paper has explored these specific relationships empirically that the authors are aware of. This work is original in terms of its assessment of questions of what second-order effects exist in predicting employee satisfaction among older workers.

Abstract

Purpose

Employee loyalty is generally a very positive trait. However, when loyal employees are confronted with dysfunctionality in the workplace the impact on their well-being can be significant. The purpose of this paper is to assess the interaction of employee loyalty and employee experience of inter-professional dysfunction in a hospital setting to predict employee job tension.

Design/methodology/approach

The paper is based on the analysis of a cross-sectional attitudinal survey of employees within a hospital setting in Australia. The authors use OLS regression and an SPSS macro (by Hayes, 2013) to assess the regions of significance of the interaction effects.

Findings

The authors find, as anticipated, significant direct effects for employee loyalty and inter-professional dysfunction on employee job stress. The authors further find significant interaction effects that suggest that highly loyal employees who experience inter-professional dysfunction also experience disproportionately high levels of job tension.

Research limitations/implications

The main research implication of this research relates to the confirmation of the presence of an interaction effect between loyalty and inter-professional dysfunction in predicting employee job stress. Further, the zone of significance analysis (following Johnson and Neyman) suggests that this effect is evident at even low levels of inter-professional dysfunction.

Practical implications

Organisations should appreciate employee loyalty but should also be aware that loyal employees are more vulnerable to the negative consequences of organisational dysfunction than are employees with limited organisational loyalty.

Social implications

The paper confirms the importance of managing organisational cooperation between groups in organisations as a precursor to positive employee outcomes.

Originality/value

This is the first paper to investigate this interaction and to apply Johnson-Neyman analysis to confirm the regions of significance for the interaction effects noted.

Abstract

Purpose

The purpose of this paper is to investigate the manner in which employees’ experience of distributive justice (DJ) moderates the impact of intrinsic and extrinsic values on role engagement (RE). RE is especially important in the healthcare setting (examined here) due to the sector’s complexity, changeability and emotionally challenging nature.

Design/methodology/approach

Using data collected from a survey of employees from a large government health district in Southeast Queensland (Australia), this study examined the determinants of RE among a group of employees working across three hospital locations.

Findings

The focus of the study was on the impact, both directly and in combination, of espoused extrinsic and intrinsic values and perceived DJ on RE. The authors identify strong direct effects from DJ on RE, and complex third-order effects for the combinations of intrinsic (IM) and extrinsic motivation and DJ in predicting RE.

Research limitations/implications

As a cross-sectional and attitudinal survey, care must be taken in relation to common-method variance. Post hoc controls were performed in relation to this.

Practical implications

DJ is important for all, and is a powerful motivator for engagement of employees reporting highly on IM. There is evidence that the most engaged employees are not those most motivated by extrinsic rewards alone, although employees who are motivated primarily by extrinsic rewards alone can be highly engaged when they experience high levels of DJ.

Social implications

For managers seeking to engage their employees, an understanding of the different motivators for intrinsically vs extrinsically inclined employees is important. Taken together, these results suggest that employee RE is driven by a complex set of factors that differ between employees. Managing this complexity is an important consideration for managers.

Originality/value

This is the first empirical study to show these interaction effects using these measures. The healthcare context, generally under researched, also features in this study.

Abstract

Purpose – Using elective egg and sperm freezing as a case to compare representations of men and women as agents of biological reproduction, this chapter aims to understand how gender and risk are co-produced in the context of new reproductive technologies (NRTs).

Methodology – Through a content analysis of newspaper articles published between 1980 and 2016 about egg and sperm freezing, the author traces how fertility risks facing men and women are portrayed in the media.

Findings – Candidates for egg freezing were portrayed in one of the three ways: as cancer patients, career women, or single and waiting for a partner. The ideal users of sperm freezing are depicted in primarily two ways: as cancer patients and as employees in professions with hazardous working conditions. Threats to future fertility for women pursuing careers uninterrupted by pregnancy and child-rearing and women seeking romantic partners are largely portrayed as the result of internal risks. However, threats to future fertility for men working in dangerous professions are largely portrayed as external to them.

Research Limitations – Race and class did not emerge as dominant themes in these data; given the lack of accessibility to NRTs by class and race, this silence must be interrogated by further research.

Value – By comparing the constructions of at-risk groups, the author argues the medicalization of reproduction is gendered as fertility risks portrayed in the media take on a different character between men and women. This research shows how the gendered construction of infertility risk reinforces normative expectations around child-rearing and perpetuates gender inequity in parenting norms.

Abstract

Purpose

This paper aims to present commentary on adult safeguarding designated officers’ (nominated senior staff members in the Health Service Executive and its funded services) in Ireland readiness for online learning and their perception of the blended approach used in their training and considers the implications of this for practice.

Design/methodology/approach

In total, 233 designated officers were surveyed. A self-administered online survey comprising of two instruments was used. The questionnaire combines two previously validated tools measuring readiness for online learning and perceptions of blended learning. Minor modifications were made to account for local circumstance and language. Demographic questions relevant to the survey population were added. The response rate was 49%.

Findings

Designated officers regarded specified competencies for online learning as important to very important with highest importance placed in time management. Designated officers expressed they were confident to very confident in their ability to accomplish all competencies in online learning. The highest confidence was in the technical competencies. Designated officers are demonstrating high readiness for online learning. Designated officers reported positively on online contributions and on blended learning workload. The scores indicate positive perceptions of the blended learning experience. Responses to an open-ended question were largely positive with some suggestions for improvement made.

Practical implications

Findings have helped inform planning and development of the Health Service Executive’s adult safeguarding training for designated officers with changes made since this survey was undertaken.

Originality/value

To the best of the author’s knowledge, this is the first time a survey was undertaken of designated officers in Ireland with a view to improving safeguarding training. Recommendations for adaptions to the blended designated officer programme were proposed and subsequently accepted and implemented.

Abstract

Purpose

The identification of the main risk triggers is essential for the hospital’s survival and performance with direct effects on its patients’ health and well-being. For this reason, in this paper some of the most important risk categories have been determined. While in a previous research a qualitative analysis has been done for determining which are the most important risks felt by the patient that are believed to affect their health through the usage of a questionnaire and through conducting a confirmatory factor analysis, the purpose of this paper is to analyze the quantitative side of these risks’ presence in a hospital.

Design/methodology/approach

On this purpose, four main categories of risks have been considered (the same as in the qualitative research) and they have been analyzed from the hospital’s point of view – through the usage of the hospital financial and internal documents. Therefore, a series of indicators have been determined for each risk category. After that, a representative indicator has been selected and the grey incidence analysis has been conducted.

Findings

By comparing the results gathered form this study with the qualitative analysis conducted among the patients (Delcea et al., 2016) it can be said that there can be seen a difference among the way a hospital and a patient perceive the risks within a medical activity. While for the hospital, the most affecting risk is the technological and hospital conditions risk, for the patients the most affecting risk seems to be the human resources and clinical risk. The mismanagement risk and inability to treat patients is the second in intensity for both the hospital and patients, with a smaller value in the patients’ case.

Practical implications

From here, the research can be extended for capturing the risks that are considered to be important for the medical stuff, which will permit us to have a global image over the healthcare risks. After that, a comparative analysis among the hospitals with different financial performance can be conducted in order to see how these risks are affecting their performance and to determine which can be the decisions that can fostering the reduction of these risks.

Originality/value

The present paper offers a quantitative analysis from the hospital’s point of view using the advantages offered by the grey systems theory. Combining this analysis with a qualitative one conducted on the patients, the managers of the hospital can a have a more adequate view over the risks that they are facing with. In this context, grey systems theory offers the needed methods for dealing with such situations.
